This position will work a hybrid schedule, which includes a combination of working at various onsite locations: La Jolla Medical Center, Hillcrest Medical Center, Encinitas, and remote.

DESCRIPTION

Under general supervision, and in addition to performing the full range of operational level duties, the Clinical Social worker in the Palliative Care Outpatient Clinical Program will be responsible for independently and with a trans-disciplinary team to conducting bio/psycho/social assessment/evaluations from a patient/family and systems-centered core perspective. The Clinical Social worker will participate in a professional team review of patient care, provide casework and group work for patients and their families, identify community resources, and assist patients and their families in connecting to and navigating these resources.

The Social Worker will individually serve as a multidisciplinary outpatient care team member with liaison responsibilities with hospital and outpatient clinic staff. This position includes facilitating family meetings, working with anticipatory grief and difficult family dynamics, and understanding placement options and appropriate social, financial, and legal services.

MINIMUM QUALIFICATIONS
  • A Master's Degree in Social Work from an accredited school.

  • Licensed Clinical Social Worker (LCSW) - issued by the state of California.

  • A minimum of three (3) years post-Master's Degree (MSW) experience. Palliative or hospice care experience is highly preferred.

  • Knowledge and abilities essential to the successful performance of the duties assigned to the position.

  • Excellent skills to communicate and influence effectively with all levels of staff, physicians, patients, and external constituents, both verbally and in writing.

PREFERRED QUALIFICATIONS
  • Advanced certification in hospice or palliative medicine.

  • Experience working alongside an interdisciplinary team.

  • Experience working with vulnerable populations.

  • Bilingual (English/Spanish).

SPECIAL CONDITIONS
  • Must be able to work various hours and locations based on business needs.
  • Employment is subject to a criminal background check and pre-employment physical.

Pay Transparency Act

Annual Full Pay Range: $92,841 - $122,315 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $44.46 - $58.58

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
